Requesting Medical Records Related to Care Received at the Clinic
Vancouver Coastal Health (VCH) maintains the medical records previously held by the Elizabeth Bagshaw Clinic.
Former clients who wish to request a copy of medical records related to care received at the Elizabeth Bagshaw Clinic, or to request records on behalf of another person, need to follow the steps below.
